Operational Risk Register — Good Practice Reference Amer Morgan · Audit Consultant & Governance AdvisorOperational Risk Register — Good Practice Reference
Central bank operational risk · five-tier rating · interactive working template● All entries below are illustrative training examples built on published international guidance. They are not the data, risks or controls of any named institution. Replace every line with your own department's facts before use.A register is a wide table: scroll it sideways to reach the control, treatment and action columns. Hover any column heading for what belongs in it, and use Edit on a row to open the guided form, where every field carries a note on what to type.
Five-by-five assessment matrix — five rating tiers
Likelihood across, impact up. Cells are not a simple product: where likelihood and impact are swapped, the higher-impact cell is never rated lower, so a rare event with catastrophic consequences never falls into a low tier. Counts show where the residual ratings of the current register sit. Click a cell to filter the register to it.

Monetary bands are deliberately left blank. Each institution sets them against its own budget, and a figure invented here would be misleading. Agree the five bands with your risk committee and write them in before the register is used.
